Surgical treatment of abdominal aortic aneurisms
Secția Chirurgie Vasculară IMSP SCR, Al XI-lea Congres al Asociației Chirurgilor „Nicolae Anestiadi” din Republica Moldova și cea de-a XXXIII-a Reuniune a Chirurgilor din Moldova „Iacomi-Răzeșu” 27-30 septembrie 2011În secția chirurgie vasculară a IMSP SCR, pe parcursul anilor 1988-2010 au fost operați 224 pacienți cu anevrisme ale aortei abdominale. SCa criteriu
diagnostic pentru anevrism s-a considerat dilatarea aortei abdominale cu două diametre şi mai mult. Vârsta pacienților a fost cuprinsă între 15 şi
89 ani. Din totalul de pacienți 79,5%(178) au fost bărbați şi 20,5%(46) femei. La 18,75%(42) pacienți anevrismele au fost complicate prin ruptură
completă sau incompletă, fiind operați în regim de urgență imediată. Etiologia a fost: ateroscleroza - 94,65% (212), aortoarteriita nespecifică - 3,57% (8),
sindromul Marfan - 1,78% (4). Diagnosticul a fost stabilit clinic, ultrasonografic, prin Duplex vascular, angio-CT, aortografie, RMN. În 3 cazuri (1,34%)
anevrismul implica şi arterele renale. Doi pacienți, neincluşi în studiu, au decedat preoperator prin hemoragie masivă cauzată de ruptura spontană a
anevrismului în duoden şi cavitatea abdominală liberă. Toți pacienții au fost supuşi rezecției anevrismului aortal cu protezare aorto-distală (aortală,
biiliacă, ilio-femurală, bifemurală), iar în 3 cazuri cu replantarea arterelor renale şi viscerale. Mortalitatea în anevrismele rupte a fost 57% (24), iar în
cele complicate - 8% (18). Concluzii: pe parcursul ultimilor 5 ani a crescut ponderea pacienților cu anevrisme simptomatice sau deja complicate prin
ruptură, astfel că toți pacienții cu factori de risc trecuți de 50 ani trebuie supuşi screening-ului prin USG sau Dopplerografie. Mortalitatea postoperatorie
în anevrismele rupte s-a micşorat cu 13 %, iar în cele operate programat cu 7,55%. Implimentarea metodei endovasculare ar permite reducerea
substanțială a acestui indice, mai ales în cazul anevrismelor complicate prin ruptură.During the period of 1988 - 2010, 224 patients underwent surgery for abdominal aortic aneurisms. The main diagnostic criteria was dilatation of
the abdominal aorta by two diameters and more. All patients were aged between 15 and 89. Of all patients 79,5% (178) were males and 20,5% (46)
were female. In 18,75% (42) cases, a complete or incomplete rupture of the aneurisms occurred. These patients underwent urgent surgery. Etiology:
atherosclerosis – 94,65% (212), nonspecific aortoarteritis – 3,57% (8), Marfan syndrome – 1,78% (4). The diagnosis was made by clinical findings, vascular
Duplex scanning, angio-CT and MRI. In 3(1,34%) cases the aneurism involved the renal arteries. There were two lethal outcomes, one caused
by spontaneous rupture of the aneurism in the duodenum and the second caused by rupture into the peritoneal cavity. These cases were not included
in the study. All patients underwent aneurism resection with aortal –distal (aortic, biiliac, ilio-femural, bifemural) allografting, while in three cases
reimplantation of the renal arteries was necessary. The mortality in cases of ruptured aneurisms consisted 57% (24), and in cases of uncomplicated
aneurisms – 8% (18).Conclusions: During the last 5 years we observed a raise in the rate of symptomatic aneurisms, including those complicated with
rupture. Aortic ultrasonography or dopplerography should be used as screening methods in all patients, with risk factors, that are aged 50 and more.
Postoperative mortality in cases of ruptured aneurisms has decreased by 13%, while in cases of planned aneurism surgery by 7,55%. Implementation
of endovascular techniques would reduce substantially these indices
The use of extraanatomic by-passes in vascular surgery

pacienților. Studiul de față are ca scop elucidarea acestor probleme folosind analiza rezultatelor a unui eşantion de 38 pacienți cu by-passuri extraanatomice
operați în perioada anilor 1987- 2010. Indicații pentru efectuarea by-passului extraanatomic a fost ischemia critică a membrelor inferioare
asociată cu patologia cardiovasculară şi pulmonară avansată la 21 de pacienți. Alt grup la care s-au aplicat by-passuri de ocolire a fost format din 6
pacienți cu procese supurative. La pacienții cu trautisme vasculare asociate cu distrugere importantă şi infectare a țesuturilor adiacente au fost aplicate
11 by-passuri extraanatomice. Rezultatele obținute depind în mare masură de patul vascular periferic. La pacienții cu ischemie critică pe fondalul
aterosclerozei obliterante, cu patologie cardiovasculară şi pulmonară avansată sau cu procese supurative se presupunea o posibilă afectare cronică a
patului periferic, astfel rezultatele fiind nu cele mai bune – aproximativ jumatate din by-passuri s-au trombozat în perioada postoperatorie precoce
ori la un an. La pacienții cu traumatisme vasculare care prezentau un pat vascular periferic permiabil, toate by-passurile erau funcționale pe toată
perioada de observație (de la 1 la 11 ani). Aşadar, şunturile extraanatomice prezintă o alternativă revascularizarilor clasice la anumite grupuri de
pacienți şi sunt unica şansă de salvare a membrelor. Aceste proceduri sunt indicate pacienților cu procese supurative, pentru ocolirea zonei afectate.
Un alt grup prezintă pacienții cu patologie cardiovasculară şi pulmonară avansată, astfel by-passurile extraanatomice find mai puțin traumatice decît
cele tradiționale. La pacienții cu traumatisme vasculare associate cu distrucții tisulare adiacente extinse şi plagi cu contaminare bacteriana severă bypassurile
extraanatomice sunt de electie. Cu siguranță aceste proceduri trebuie să le posede oricare chirurg vascular.Until now there are no precise criteria regarding the surgical management of extraanatomic by-passes. This incudes indications for surgery,
by-pass application techniques and patient selection. The aim of the study is to elucidate this problem, by analyzing the outcomes of 38 patients
with extraanatomic by-passes. All patients underwent surgery between 1987-2010. Critical ischaemia of the lower limbs associated with
advanced cardiovascular and pulmonary pathology served as indication for the use of extraanatomic by-passes in 21 patients. Another group of
6 patients operated with extraanatomic by-passes had septic processes and 11 patients with vascular injuries associated with bacterial contamination
and delabrante wounds. The results mostly depended on the peripheral vascular bed. In patients with critical ischaemia associated with
atherosclerosis and advanced cardiovascular and pulmonary disease, or septic processes, the peripheral vascular bed was chronically affected,
thus negatively influencing the results – thrombosis of the by-passes in the early postoperative period or after one year occurred in one half of
patients. In patients with a satisfactory runoff, all by-passes were patent through all the observation period (from 1 to 11 years). In conclusion
the extraanatomic by-passes represent an alternative choice for classical revascularization surgery in some groups of patients, and are the only
solution for limb salvage. These interventions are indicated in patients with suppurative processes, to avoid the septic focus. Another group
consists of patients with associated advanced cardiovascular and pulmonary pathology, thus the extraanatomic by-passes being less traumatic
then the traditional ones. Extraanatomic by-passes are also indicated in cases of vascular injuries associated with bacterial contamination and
delabrante wounds. Certainly these are procedures that every vascular surgeon must possess
Revascularization of the infrapopliteal arterial segment in occlusive-stenotic processes
Republica Moldova, IMSP SCR, FPM, Clinica de chirurgie, Secția chirurgie vasculară, Al XI-lea Congres al Asociației Chirurgilor „Nicolae Anestiadi” din Republica Moldova și cea de-a XXXIII-a Reuniune a Chirurgilor din Moldova „Iacomi-Răzeșu” 27-30 septembrie 2011Scopul: Studierea rezultatelor precoce ale revascularizărilor chirurgicale a segmentului arterial infrapopliteu în procesele ocluziv-stenotice. Materiale
şi metode: Pe parcursul anilor 1995-2010 în secția de Chirurgie Vasculară s-au efectuat 566 de intervenții chirurgicale la 533 pacienți,
dintre care 33 la ambele membre inferioare. La 82% pacienți s-a determinat gr III -IV de ischemie la nivelul membrelor inferioare, fapt ce prezintă
indicație absolută pentru a interveni chirurgical. Tactica şi volumul operator a fost apreciat conform rezultatelor investigațiilor clinice, duplex
scanare, angiografie, CT angiografie şi explorare intraoperatorie. Intervențiile de preferință efectuate au constituit, by-passurile cu grefon safen
intern inversat şi trombendarterectomiile din arterele poplitea şi tibială cu petic din autovenă. În 68 (12%) cazuri au fost revascularizate şi segmentele
proximale necesitînd by-pass aorto-femural sau femuro-popliteu respectiv. Rezultate: În perioada postoperatorie precoce, retrombozele
au survenit în 73 (12,9%) cazuri, dintre care la 28 pacienți datorită reintervențiilor precoce am obținut rezultate favorabile. La 37 pacienți s-a
recurs la amputarea coapsei sau gambei. Concluzie: Conform datelor noastre, revascularizarea segmentului arterial infrapopliteu în procesele
ocluziv-stenotice este posibilă în 92,5% cazuri. Intervențiile de preferință conform indicațiilor şi investigațiilor efectuate, au constituit by-passurile
şi trombendarterectomiile cu folosirea materialului autolog.Aim of the study: Evaluation of early results of surgical revascularization of the infrapopliteal arterial segment in the occlusive-stenotic processes.
Material and methods: During the period of 1995-2010 in the Department of Vascular Surgery there have been performed 566 operations in
533 patients, of which 33 for both lower limbs. Critical ischemia was identified in 82% of patients, determining absolute indications for surgery.
Further surgical tactics were appreciated after physical examination, duplex scan, angiography, CT angiography and intraoperatory exploration.
The elective surgical procedure was by-pass with reversed autologous internal saphenous graft and thrombendarterectomy from the popliteal
and tibial arteries with autovenous patch. Proximal segment revascularization using aorto-femoral or femuropopliteal by-passes were necessary
in 68 (12%) of cases. Results: The early postoperative period was complicated by rethrombosis in 73 (12,9%) cases. Early reintervention
has been performed to 28 patients with satisfactory results. In 37 cases high amputations were necessary. Conclusion: Revascularization of the
infrapopliteal arterial segment in occlusive-stenotic processes was possible in 92,5% of cases. The surgical procedures of choice were by-passes
and thrombendarterectomy with use of autologous material
Reconstruction of inferior vena cava in cases of renal cancer metastatic caval wall invasion
Republica Moldova, IMSP SCR, Secția chirurgie vasculară, Al XI-lea Congres al Asociației Chirurgilor „Nicolae Anestiadi” din Republica Moldova și cea de-a XXXIII-a Reuniune a Chirurgilor din Moldova „Iacomi-Răzeșu” 27-30 septembrie 2011În cazul tumorilor renale maligne, nu rareori (4%-10%) este implicată vena cavă inferioară, fapt ce prezintă o barieră serioasă pentru intervențiile
chirurgicale radicale. Acest studiu analizează rezultatele tratamentului chirurgical a 15 pacienți (14 cazuri de rinichi drept si 1 de rinichi stîng) cu
concreştere tumorală în vena cavă inferioară, cu tromb metastatic în vena cavă şi cu metastaze în ganglionii paracavali si paraaortali. Diagnosticul
preoperator a fost înfăptuit prin USG, Duplex scanare, CT angiografie şi angiografie. Era preferabilă embolizarea preoperatorie de arteră renală. La toți
pacienții a fost efectuată nefrectomia, care includea şi limfodisecția retroperitonială. La toți pacienții s-a efectuat restabilirea continuității venei cave
inferioare prin diferite metode: înlaturarea trombului metastatic cu suturarea venei cave, rezecția venei cave cu restabilirea ei ulterioară prin protezare
sau petic. La 13 pacienți invazia celulelor blastice în peretele venei cave inferioare a fost confirmată histologic postoperator. A decedat intraoperator
un pacient la care a fost folosit circuitul extracorporeal, pentru înlăturarea trombului metastatic prelungit pînă la atriul drept. Cauza decesului a fost
sindromul de coagulare intravasculară diseminată. Perioada postoperatorie la 14 pacienți a decurs fără complicații majore şi s-au externat în stare
satisfăcătoare. Datele obținute ne denotă faptul că pentru un tratament cît mai efectiv a acestor tumori, logic şi rațional este de a efectua rezecția
segmentului de venă lezat cu reconstrucția ulterioară cînd este posibil. Avansarea tehnicii chirurgicale şi rezultatele pozitive obținute ne permit de a
promova această tactică agresivă de tratament.In patients with malign renal tumors, inferior vena cava is involved in 4-10%, thus representing a serious barrier for radical intervention. The
aim of this study is to analyze the results of surgical treatment of 15 patients (in 14 cases the right kidney was involved and in one case the left
one), with metastatic caval wall invasion, metastatic thrombus in the inferior vena cava, and metastasis of paracaval and paraaortal lymph nodes.
The preoperative diagnosis was made using ultrasonnography, duplex scanning, CT angiography and angiography. Preoperative renal artery
embolization was preferable. All patients underwent nefrectomy with retroperitoneal lymph nodes dissection. The reconstruction of the inferior
vena cava was made by several methods: removement of the metastatic thrombus with caval suturing, resection of vena cava with grafting or
patching. In 13 cases the tumoral caval wall invasion was demonstrated by postoperative histology analysis. There has been one intraoperative
lethal case in a patient that had a cardiopulmonary by-pass system applied for removement of a metastatic thrombus that has reached the right
atrium. The cause of death was disseminated intravascular coagulation. In 14 cases the postoperative outcome was satisfactory, without major
complications. In conclusion, the achieved results indicate that resection of the affected cava with its grafting is the elective method for a more
effective treatment of these tumors
Profundo-aortal index: profundal revascularization efficiency prognosis criteria
Spitalul Clinic Repulican, secția angiochirurgie, Al XI-lea Congres al Asociației Chirurgilor „Nicolae Anestiadi” din Republica Moldova și cea de-a XXXIII-a Reuniune a Chirurgilor din Moldova „Iacomi-Răzeșu” 27-30 septembrie 2011Scopul lucrării: Ameliorarea rezultatelor tratamentului chirurgical al pacienților cu ischemie critică cauzată de afecțiuni aterosclerotice polisegmentare
a arterelor membrelor inferioare. Material şi metode: Studiul a fost efectuat pe un lot de 400 de pacienţi cu afecțiuni arteriale aterosclerotice polisegmentare
cu ischemie critică a membrelor inferioare, care au fost tratați în sectia chirurgie vasculară a IMSP „Spitalul Clinic Republican” între anii
2001-2005 (lotul de studiu) şi a unui grup de 350 pacienți, tratați în chirurgia vasculară între anii 1998-2000 (lotul martor). Majoritatea pacienților aveau
între 45 şi 65 ani. Sexul masculin covârşitor predomină. Etiologic la toți pacienții a fost stabilită ateroscleroza. Toţi pacienţii au fost investigaţi clinic,
prin Duplex scanarea sistemului arterial, arteriografie şi angio CT. La fel s-a determinat indicele maleolo-brahial şi indicele profundo-aortal pentru
fiecare pacient. Indicele profundo-aortal constituie raportul dintre suma diametrului porțiunii istmice a arterei femurale profunde, diametrului extern
a porțiunii permeabile a arterei femurale profunde, diametrului intern a porțiunii permeabile a arterei femurale profunde şi suma dintre diametrul
aortei abdominale la nivelul arterelor renale şi diametrul aortei la bifurcație.Toţi pacienţii au suportat intervenţii de revascularizare prin bazinul arterei
femurale profunde. Rezultatele tratamentului chirurgical: S-a micşorat considerabil rata amputațiilor înalte după reconstrucții arteriale, constituind 5% în
lotul de studiu contra la 8,8% în lotul martor. În grupul de pacienți cu valoarea indicelui profundo-aortal mai mare de 0,5 n-a survenit nici o amputație.
În grupul de pacienți cu valoarea indicelui profundo-aortal 0,31-0,5 au fost efectuate numai 5 amputații înalte. Majoritatea amputațiilor (15) au fost
necesare la pacienții, avînd valoarea indicelui profundo-aortal mai mic decît 0,3. În acest grup este necesară efectuarea în aceiaşi şedință operatorie şi
a unui pontaj femuro-distal. Concluzii: Indicele profundo-aortal poate fi folosit cu succes pentru prognosticul eficacităţii revascularizărilor profundale.Aim of the study: Improvement of the surgical treatment of patients with critical ischemia caused by atherosclerotic polisegmentary lesions of lower
limb arteries. Materials and methods: The study included a group 400 patients with polisegmentary arterial atherosclerotic lesions of the lower limbs
with critical ischemia that underwent surgery during the period of 2001-2005 (the study group) compared to a second group of 350 patients operated
during the period of 1998-2000 (witness group). Most of the patients were aged between 45 and 65 years. Males were in majority. Atherosclerosis was
found in all patients. All patients were investigated clinically, by Duplex scanning, angiography, and angio-CT. Maleolo-brachial and profundo-aortal
indices were determined in all patients. Profundo-aortal index represents the ratio between the sum of isthmic diameter of deep femoral artery, the
outer diameter of the permeable segment of deep femoral artery, the inner diameter of the permeable segment of deep femoral artery and the sum of
the abdominal aortic diameter at renal artery emerging level and the bifurcation aortic diameter. All patients underwent surgical revascularization
using the deep femoral artery. Results of the surgical treatment showed a significant decrease in the number of high limb amputations, that is by 5 %
in the study group and by 8,8% in the witness group. In the group of patients with the profundo-aortal index higher than 0,5 no amputations were
observed. In the group of patients with the index ranging between 0,31 and 0,5, only 5 high amputations were performed. Most amputations (15) were
necessary in patients having the index lower than 0,3. In this group, a femuro-distal by-pass is recommended in the same surgical session. Conclusions:
The profundo-aortal index can be successfully used for deep femoral artery revascularization efficiency prognosis
Particularities in the treatment of vascular trauma

Materiale şi metode: Pe parcursul anilor
1990-2010 au fost tratați cu plagă deschisă 66 pacienți cu traumatisme vasculare asociate cu plăgi delabrante şi contaminare bacteriană importantă.
La 12 pacienți s-au aplicat by-passuri extraanatomice cu grefon safen intern inversat, plaga inițială fiind lăsată deschisă pentru drenare
adecvată şi debridări repetate după necesitate. La 54 pacienți by- passul extraanatomic nu a fost posibil din considerente tehnice, diametrul şi
lungimea insuficientă a safenei interne pentru grefon, distrugerea şi contaminarea considerabilă a țesuturilor regiunii cu posibilitatea de trecere
a by-passului extraanatomic. Pentru aceşti pacienți s-a recurs la repararea vasculară cu plagă deschisă. Rezultate: În perioada postoperatorie
au survenit 2 hemoragii erozive, care au fost stopate cu ajutorul aplicării peticelor autovenoase, drenării şi suturării plăgilor deja granulante.
Intervenție de ligaturare a arterelor şi amputații nu au fost necesare. Concluzie: Intervenția de elecție în traumatismele vasculare asociate cu
distrugeri extinse şi contaminare bacteriană importantă a țesuturilor adiacente sunt by-passurile extraanatomice efectuate pe țesuturile viabile
necontaminte. În cazul imposibilității aplicării by-passului este solicitată metoda de revascularizare in situ cu plagă deschisă. Accesul pentru
controlul şi debridările repetate a țesuturilor adiacente vasului reparat, este realizat prin plaga postoperatorie nesuturată sau prin contraincizii
suficient de largi.Aim of the study: Assessment of the possibility of surgical treatment of vascular trauma using the open wound method. Material and methods:
During the period of 1990-2010, 66 patients with vascular injuries associated with bacterial contamination and delabrante wounds were treated
using the open wound method. Extraanatomic by-passes with reversed internal saphenous graft has been aplied to 12 patients. The initial
wound was left opened for proper drainage and repeated debridement. In 54 cases the extraanatomic by-pass was not possible for such technical
reasons as insufficient diameter and length of the autologous saphenous graft, considerable tissue destruction and contamination in the region
with opportunity to pass the graft. In these patients open wound vascular repair was used. Results: During the postoperative period 2 cases of
erosive bleeding occurred, which were stopped by applying autovenous patches, adequate wound drainage and suturing on granulation tissue.
Such interventions as arterial ligation and amputations were not necessary. Conclusion: In cases of vascular trauma associated with extended
damage and important bacterial contamination of the adjacent tissue,it is preferable to perform extraanatomic by-passes within viable and
uncontaminated tissues. In cases when the by-pass cannot be performed, revascularization in situ using the open wound method is required.
Access for control and repeated debridement of the tissues ajacent to the repaired vessel is realized through the unsutured postoperative wound
or through large additional contraperture incisions
EPIdemiology of Surgery-Associated Acute Kidney Injury (EPIS-AKI) : Study protocol for a multicentre, observational trial
More than 300 million surgical procedures are performed each year. Acute kidney injury (AKI) is a common complication after major surgery and is associated with adverse short-term and long-term outcomes. However, there is a large variation in the incidence of reported AKI rates. The establishment of an accurate epidemiology of surgery-associated AKI is important for healthcare policy, quality initiatives, clinical trials, as well as for improving guidelines. The objective of the Epidemiology of Surgery-associated Acute Kidney Injury (EPIS-AKI) trial is to prospectively evaluate the epidemiology of AKI after major surgery using the latest Kidney Disease: Improving Global Outcomes (KDIGO) consensus definition of AKI. EPIS-AKI is an international prospective, observational, multicentre cohort study including 10 000 patients undergoing major surgery who are subsequently admitted to the ICU or a similar high dependency unit. The primary endpoint is the incidence of AKI within 72 hours after surgery according to the KDIGO criteria. Secondary endpoints include use of renal replacement therapy (RRT), mortality during ICU and hospital stay, length of ICU and hospital stay and major adverse kidney events (combined endpoint consisting of persistent renal dysfunction, RRT and mortality) at day 90. Further, we will evaluate preoperative and intraoperative risk factors affecting the incidence of postoperative AKI. In an add-on analysis, we will assess urinary biomarkers for early detection of AKI. EPIS-AKI has been approved by the leading Ethics Committee of the Medical Council North Rhine-Westphalia, of the Westphalian Wilhelms-University Münster and the corresponding Ethics Committee at each participating site. Results will be disseminated widely and published in peer-reviewed journals, presented at conferences and used to design further AKI-related trials. Trial registration number NCT04165369
Diagnosis and management of a leg chronic posttraumatic arteriovenous fistula

fistulelor arteriovenoase (FAV). Complicaţiile tardive ale FAV netratate pot fi:dilatarea arterială proximală,
congestia venoasă, insuficienţa cardiacă cronică şi ischemia membrului. Prezentăm cazul unui pacient de 59 de
ani, care a suportat un traumatism prin armă de foc (schrapnell) a abdomenului şi membrelor inferioare cu 8 ani
în urmă, fără clinică de leziuni vasculare. Acuzele la spitalizare au fost durerea inghinală pe dreapta, asimetria
coapsei şi dispnee la efort moderat. CT-angiografia a fost sugestivă pentru prezenţa FAV iliace și poplitee.
Angiografia DSA confirmă doar FAV poplitee la nivel P1. A fost efectuată protezarea arterei cu aloproteză
sintetică ePTFE şi suturarea fistulei prin acces venotomic. Lichidarea FAV a fost confirmată postoperator prin
angiografie DSA. Efectuarea angiografiei DSA este recomandată pentru stabilirea corectă a volumului operator
şi confirmarea rezultatului postoperator.Penetrating trauma of the extremities with asymptomatic blood vessels damage can cause arteriovenous fistulas
(AVF). The late complications of the untreated AVF can be: proximal arterial dilatation, venous congestion,
chronic heart failure and limb ischemia. We present a 59 years patient, who suffered 8 years ago an abdomen and
both legs schrapnell injury without vascular lesion symptoms. His complaints at the moment of hospitalization
were : pain in the right groin, thigh asymmetry and moderate dyspnea on exertion. CT-angiography was
suggestive for the presence of both iliac and popliteal AVFs . DSA angiography confirmed only the popliteal
AVF at the P1level. It was performed an ePTFE graft repair of the popliteal artery and the AVF suturing through
the popliteal vein. The AVF liquidation was confirmed by a postoperative DSA angiography. Preoperative and
postoperative DSA angiography is recommended in the AVF management
EPIdemiology of Surgery-Associated Acute Kidney Injury (EPIS-AKI): Study protocol for a multicentre, observational trial
Introduction More than 300 million surgical procedures are performed each year. Acute kidney injury (AKI) is a common complication after major surgery and is associated with adverse short-term and long-term outcomes. However, there is a large variation in the incidence of reported AKI rates. The establishment of an accurate epidemiology of surgery-associated AKI is important for healthcare policy, quality initiatives, clinical trials, as well as for improving guidelines. The objective of the Epidemiology of Surgery-associated Acute Kidney Injury (EPIS-AKI) trial is to prospectively evaluate the epidemiology of AKI after major surgery using the latest Kidney Disease: Improving Global Outcomes (KDIGO) consensus definition of AKI. Methods and analysis EPIS-AKI is an international prospective, observational, multicentre cohort study including 10 000 patients undergoing major surgery who are subsequently admitted to the ICU or a similar high dependency unit. The primary endpoint is the incidence of AKI within 72 hours after surgery according to the KDIGO criteria. Secondary endpoints include use of renal replacement therapy (RRT), mortality during ICU and hospital stay, length of ICU and hospital stay and major adverse kidney events (combined endpoint consisting of persistent renal dysfunction, RRT and mortality) at day 90. Further, we will evaluate preoperative and intraoperative risk factors affecting the incidence of postoperative AKI. In an add-on analysis, we will assess urinary biomarkers for early detection of AKI. Ethics and dissemination EPIS-AKI has been approved by the leading Ethics Committee of the Medical Council North Rhine-Westphalia, of the Westphalian Wilhelms-University Münster and the corresponding Ethics Committee at each participating site. Results will be disseminated widely and published in peer-reviewed journals, presented at conferences and used to design further AKI-related trials. Trial registration number NCT04165369.
